"On the Spot" Super Star Award

A spot award is a spur-of-the-moment employee recognition for employee performance and exemplary behavior as and when it happens. To recognize employees for their day-to-day efforts that contribute in a special way to getting the job done. Peer-to-Peer, managers or supervisors may give immediate recognition to recognize and reward employee excellence.

"Together We Can" Employee of the Month

Employee of the Month 

As a company we highly value our employees efforts, contributions and commitment to their jobs, but also consistent excellence towards co-workers, to partners and to clients by providing high quality support services. We want to recognize and reward employees who consistently demonstrate "above and beyond" contributions that resemble and reflect with our company's mission, core values and guiding principles.

 

Rules & Eligibility

The Employee of the Month award program was developed to encourage recognition of Eleos Family Services staff who have significantly benefited the company, clients and partners and made contributions beyond the usual expectations of their employment.

 

Award

The Employee of the Month will receive a plaque and a cash award of $100.00. The EOM will receive a "spotlight feature" on our internal and external sites, as well as on our social media platforms. They will be a candidate for the Employee of the Year Award, which is will be announced in Q1 of the following year. Each Employee of the Year is presented a plaque and a check for $300.00.

 

Eligibility and Rules 

To be eligible for the Employee of the Month award, an employee must have held a full-time position for at least six (6) consecutive calendar months. Nominations not selected as Employee of the Month will be held and reconsidered each month for one year from the date first received. Employees who have retired or resigned from the company since being nominated are not eligible to receive the award. Nominee must not have received the Employee of the Month award in the past three (3) months.

 

Suggested Criteria for Nomination/Selection

Employee of the Month Selection Committee nominee selection will be based upon the following criteria. However, an employee need not meet all these requirements to receive the award. In order for the committee to fully evaluate the nominated employee's attributes and/or achievements, it is important that specific examples are cited in a nomination.

  • Does the employee have an outstanding attendance record?
  • Does the employee consistently "go the extra mile" without being asked?
  • Is the employee creative within their position? Have they improved work methods, improved efficiency within the department, contributed to new ideas, or enhanced their own responsibilities?
  • Do they advance their knowledge by learning new skills?
  • Is the employee cooperative, compassionate, and friendly to fellow employees and clients and their families on a regular basis?
  • Have they volunteered for special projects?
  • Does the employee have better-than-average ability to work under pressure?
  • Is the employee extraordinarily loyal and dedicated to Eleos Family Services?
  • Does the employee possess other qualities that would make them deserving of such an award?

Remember - the employee does not have to meet all criteria in order to be nominated or selected for the Employee of the Month award.

1)  Employee of the Month nomination form

Our Employee of the Month nomination form is a document that allows supervisors, managers, or other employees to nominate individuals for our "TWC" Together We Can Employee of the Month award. This award is typically used to recognize employees who have demonstrated outstanding performance, made exceptional achievements, or gone above and beyond in their job duties over a specific period of time. The Employee of the Month nomination form helps us gather and organize nominations, so we can select a winner.

 

2)  Purpose Employee of the Month Form

The purpose of an Employee of the Month nomination form is to help us collect nominations from our employees easily and store them in one central location. Using an online form eliminates the need to gather nominations via paper or email and helps evaluators sort through them quickly and efficiently. The Employee of the Month Nomination Form helps us to reward our hard-working employees to make sure they know we recognize their contributions and efforts.

 

3)  Employee of the Month Selection Process

The selection process is completed by Employee of the Month Selection Committees.  We accept nominations for the award at anytime. Our Employee of the Month Selection Committees will consider all entries against our company’s selection criteria for the award, then choose a recipient and present them with the award.

 

4)  Employee of the Month Selection Committee 

The Committee will be comprised of staff representing staff workers, administrative, management and executive management. Committee members are eligible to submit, receive nominations and receive EOM awards.

"Rising Star" New Hire

This award is to recognize newly hired employees who have "hit the ground running" in making a significant impact to our company, team members, youth, adolescents, individuals and their families. To be eligible newly hired employees will need to be within their first 90 days of employment at the time of the submission.

"Luminary" Leader of the Year

What does it mean to be a "Luminary" Leader ?

  • An excellent role model.
  • Inspires others to work collaboratively and creatively.
  • Promotes a work culture that is respectful, collegial and supportive. 

It doesn't matter how you define a leader, they could be a executive manager or a supervisor. This award recognizes a variety of leadership skills, including strategic thinking, communication, collaboration, decision-making, innovation, empathy, integrity, and inclusivity.

"Stellar" Employee of the Year

An Employee of the Year award is a badge of honor given to employees to recognize their efforts and contributions toward propelling the company's mission & vision and consistently exhibiting our core values and guiding principles. EOTY nominees have distinguished themselves in their overall disposition, attitude, and performance.  Nominees possess an exemplary work ethic that is both motivating and worth emulating.
